Fixation Techniques for Intraarticular Proximal Humeral Fractures

Background. The most severe type of injuries the proximal epiphysis humerus are intraarticular fractures. One main complication is development avascular osteonecrosis, which caused by peculiarities blood supply humeral head and its compromised vascularization as a result trauma. Current osteosynthesis techniques for fractures (PH) do not reduce risk osteonecrosis (AONHH) nonunion. To prevent ischemic changes in head, with reparative osteogenesis stimulation recommended.
 Aim study to specify indications various fixation humerus.
 Methods. enrolled 48 patients AO/ASIF 11C1 11C2 PH requiring surgical treatment. All were allocated into 2 groups. Retrospective (control) group included 25 who treated using locking plate or intramedullary nails. Prospective (main) 23 additionally vascularized musculoskeletal graft from coracoid process scapula transplanted fracture area.
 Results. Functional treatment results underwent surgery grafts processes (71.50% excellent 14.3% good) better than those control (35.28% 17.64% good). Consolidation occurred 92% cases (23 patients); remaining 8% (2) had pseudoarthrosis anatomical neck developed within 6 months after surgery. In group, consolidated all patients.
 Conclusion. Fractures no damage bicipital groove should be considered an indication performing without grafting. Locking case makes it much more difficult tubercular area. decreases rigidity fragments, may contribute their secondary displacement.
